| All AT17 forms MUST be submitted to the FHSAA office by Monday, July 30, 2018. The form is an affidavit of compliance for the required NFHS education courses regarding heat illness, concussions and sudden cardiac arrest. The form MUST include the signature from the athletic director and principal. Forms are to be submitted to amoseley@fhsaa.org.

The FHSAA staff will communicate multiple layers of information for member schools to include best practices, appropriate resources, frequently asked questions and more. This information will be communicated thoroughly to member schools prior to July 30, 2018, the first day of practice for fall sports. |
